Basketball Training Program Registration
1. Registration & Enrollment
By submitting the registration form, you agree to provide accurate and complete information. Enrollment in the basketball training program is subject to availability and confirmation by the academy.
The academy reserves the right to accept or decline any registration at its discretion.
2. Medical Fitness & Health Declaration
The participant (or parent/guardian) confirms that the participant is physically and medically fit to take part in basketball training sessions.
Any existing medical conditions, injuries, or allergies must be disclosed prior to participation. The academy shall not be held responsible for any health issues arising from undisclosed conditions.
3. Assumption of Risk & Liability Waiver
Participation in basketball training involves physical activity and inherent risks, including but not limited to injury, falls, collisions, or accidents.
By registering, you acknowledge and accept these risks. The academy, its coaches, and staff shall not be held liable for injuries, accidents, loss, or damage occurring during training sessions, except in cases of proven negligence.
4. Code of Conduct
All participants are expected to:

Maintain appropriate behavior at all times
The academy reserves the right to suspend or terminate participation for misconduct, without refund.
5. Fees & Payments
Program fees must be paid according to the selected plan.
Payments are non-transferable unless otherwise stated.
Failure to complete payment may result in suspension from training sessions.
6. Refund & Cancellation Policy
Registration fees are non-refundable unless the academy cancels the program.
Missed sessions due to personal reasons, illness, or scheduling conflicts are generally non-refundable unless otherwise specified by the academy.
7. Schedule Changes
The academy reserves the right to modify training schedules, coaches, or venue when necessary. Participants will be informed of any changes in advance whenever possible.
8. Media Consent
The academy may take photographs or videos during training sessions for promotional and marketing purposes (including website and social media).
By registering, you grant permission for such media use unless written notice is provided stating otherwise.
9. Personal Belongings
The academy is not responsible for lost, stolen, or damaged personal items brought to training sessions.
